JP Soccer Participant Waiver

Welcome!
Before participating in any programs, events, birthday parties, camps, or activities at our facility, a waiver must be completed for each child. This waiver must be completed by a parent or legal guardian for all participants under 18 years of age. By completing the form below, you acknowledge and accept the terms of participation and understand the inherent risks associated with physical activity. This waiver applies to all activities and events hosted at our facility and remains on file for the current year unless otherwise noted.

Please complete the form in full and ensure all information is accurate. If you have multiple children attending, a separate waiver must be submitted for each participant.
